Contingency for ProSolo and National Tour

I have a question regarding what needs to be sent in to qualify for the various contingecy awards.

Do I just have to fill out the following:

2006 Solo National Tour Contingency Registration Form (http://www.scca.com/_FileLibrary/File/2006%20Solo%20National%20Tour%20Contingency%20Part icipation%20Form.pdf)

W-9 form (http://www.scca.com/_FileLibrary/File/w9.pdf)

or are there additional forms I need to submit?

I noticed that Mike Neary was registered for the Walnut Ridge ProSolo and Branden Burkhart was registered for the Walnut Ridge National Tour.

I'm pretty sure you have to fill out a contingency for each one you plan to participate in, whether it be Kumho, Hawk, etc, in addition to the standard SCCA form. When we did one for Hoosier, I filled out a contingency form online and they sent decals, etc and such to use.
